  2. Print and fill out each form, if you have multiple dogs, each dog must have their own separate completed forms
  3. Contact your vet for filling out any information you are uncertain about
  4. Call us at (513) 221-7877 to schedule your first visit. First visit is by appointment only.

RULES, REGULATIONS & POLICIES

HEALTH: Owners will need to verify that their dogs are in good health and have not been ill in the last 30 days. Dogs who have been ill with a communicable condition in the last 30 days will require veterinarian verification of health to be admitted or re-admitted. Dogs cannot be coughing or sneezing when dropped off.

SHOTS: Dogs must be up-to-date on ALL vaccinations. Owners must submit written proof from their veterinarian. Vaccines will include a Distemper, Parvovirus combination of your veterinarian’s choice, Rabies and Bordetella ( kennel cough) every six months is preferred. Also Flu vaccine is strongly recommended in the winter. Dogs must be year-round monthly Heartworm Prevention, i.e. interceptor, Heartgard, sentinel, Trifexis, etc. They must also be on year-round monthly Flea Protection, i.e. Nexgard, Credelio, Vectra, Bracecto.

AGE: All Puppies must be at least 12 weeks old to start and be spayed or neutered by six(6) months old. Younger Puppies must have a vets approval.

BEHAVIOR: All dogs must be non-aggressive and not protective of food or toys. Owners will need to certify yhat their dog(s) have not harmed or shown any aggressive or threatening behavior towards any person or other dog.

RESERVATIONS: A reservation is only required for the first day, evaluation visit. Evaluating dogs must be dropped off between 6;00 & 7:00 am. Evaluations must be scheduled in advance.

Paperwork: All dogs must have completed, up-to-date and approved paperwork on file. There is a $23.00 non-refundable evaluation/trial day fee.

ARRIVAL: All new dogs must arrive on a leash. If the leash is left with us, the dogs name must be written on it. To avoid injury pinch collars cannot be left on in the play room. A regular collar that we may put their name on will be required.

Days and Hours: We are open Monday thru Saturday from 6:00 AM to 8PM. Special arrangements for early drop-off or late pick-up may be made with proper advance notice. Additional charges may apply.
